Behavioral Health Home Plus

Behavioral Health Home Plus (BHHP) is a comprehensive model that addresses physical health and wellness as part of recovery for adults living with serious mental illness, individuals with opioid use disorders, and adolescents participating in various behavioral health services. BHHP enhances the capacity of behavioral health providers to help people identify physical health and wellness challenges and activate people to become better informed and effective managers of their overall health. Over 60 behavioral health providers implement the BHHP model. Advanced providers participate in population health learning collaboratives focused on tobacco cessation and hypertension.
